To examine the distribution of patients with and without access to palliative care by state/region and assess geographic variation in patients with cancer with high needs of palliative care. The 2010 National Health Interview Survey Adult Components (NHIS-AC) was used to examine the medical history of the participants aged 18 years or older. Responses were used to calculate a need index for each respondent. Patients with a need index >0.6 were classified as having high needs. Descriptive analysis was used to examine the state/region of residence and the percentage of cancer patients with high needs of palliative care by state/region in 2010. Significance was set at 0.05. A total of 7,722 participants with cancer in the last year were included in the analysis. An estimated 37.35 million (47.6%) participants with cancer were without access to palliative care, and 5.43 million (7.0%) participants with cancer had high needs for palliative care. The high needs varied across regions, ranging from 5.7% in the West to 8.4% in the Northeast. States with high needs were Wisconsin (19.8%), Kentucky (20.3%), and Louisiana (22.2%). A significantly higher proportion of cancer patients without access to palliative care lived in the Midwest, compared to the Northeast, South, and West. Region was a predictor of access to palliative care (p = 0.02). The states with high needs of palliative care varied across regions. States with the highest high needs for palliative care were Wisconsin, Kentucky, and Louisiana. The proportion of cancer patients without access to palliative care varied by region, with the Midwest having the highest rate of patients without access to palliative care.
